2. There are a few different ban types grouped into "other" here - I'll mention some: CC Fraud, account sharing, account hacking and "repeated warnings or in-game violations" not covered under any of the other types of bans.

Yep, this is more or less correct. "ISK Spammers" are in-game characters spamming chat, eve mails and whatnot with ISK Selling ads.
The "ISK Seller" bans are for accounts involved in selling of ISK - typically these will be distribution accounts, middle-men or suppliers.
The graph is based on "ban types" which is what these accounts would see when attempting to log in. The "Macro Use" bans includes all macro bans; but this ban type in particular could be a graph of its own, as we differentiate between types of bots - RMT bots, player bots, chat spam bots (outside of the ISK Spamming field) and so forth.
It's also worth mentioning that we do not limit punishment to the offending account(s) - we take action against all accounts found to belong to the offender. This is a big reason not to share your accounts - your friend might buy some ISK or bot a little, and you could end up being guilty by association.

Yes absolutely. Reviewing our policy pages and providing a central location for all of these policies on our to-do list. No ETA, but we are working on it. I'd like an eveonline.com/security page, personally!
